How do kickboxers take a hard sucker knee without injury?

A knee is definitely stronger than a punch, so I am not sure which is more painful: a sucker punch (unflexed) or a prepared knee (flexed).

But a sucker knee is like the worst of both worlds - the giver probably used a lot of strength even if not full force trying to win by KO - how does the receiver not end up in hospital when his insides are smushed after getting caught off guard like this?
